Vitality 365
Seniors were showing up to doctor appointments with no record of how they had actually been feeling.

The problem
Seniors were walking into doctor appointments with no record of how they had actually been feeling. No sleep data, no symptom patterns, no medication history. Physicians were working from memory and guesswork, and patients were drawing blanks when asked the most basic question: "How have you been?"
Existing health apps were built for biohackers and fitness enthusiasts — too complex, too data-heavy, too focused on wearables. What the 60+ adult needed was something different: a simple daily habit that produced one useful output, a single page their doctor could actually read.
The gap was not technology. It was design. Nobody had built a health tracker around the doctor's appointment.
What we built
A full-stack health companion web app designed specifically for adults 60+ and the family members who support them. The entire product is built around one output: a one-page Doctor Summary PDF that consolidates 30 days of data into a single printable report a patient can hand to their doctor at the door.
Daily check-ins
Five questions, two minutes, every morning. How did you sleep? Any pain? How is your energy? How is your balance? How is your mood? Thirty days of answers become the raw material for the Doctor Summary. The habit is designed to be as frictionless as a morning coffee ritual.
Health measurements
Blood pressure, glucose, weight, SpO2, cholesterol, temperature, and heart rate. Seven measurements, one simple log, full history charts that a doctor can actually read. No wearables required. Log when you take them, see the trend over time.
The Doctor Summary PDF
One tap generates a one-page PDF. It includes check-in trends, medication adherence, health measurements, and the specific symptoms the patient wants to discuss. It is designed to be handed to a physician at the start of a visit — not emailed, not uploaded to a portal, just handed over. The appointment changes immediately.
Family sharing
Share a simple daily status with up to four family members. They see that you checked in. They do not see what you journaled. The patient stays in control. The family stops worrying. No group texts required.
The public site
A conversion-focused marketing site designed for two audiences simultaneously: the senior who will use the app, and the adult child who will set it up for them. Pricing, comparison table, testimonials, and a gifting flow for family members who want to cover the Pro plan.
The result
Within the first weeks of use, members reported their appointments felt more productive and their doctors were asking better questions. The product delivered on its core promise: show up prepared, walk out heard.
5
questions per check-in
30
days of data per summary
2 min
daily habit time
"I used to walk into my doctor's office and draw a blank when she asked how I'd been feeling. Now I hand her a one-page summary and she actually reads it. Best appointments I've ever had."
The stack
Mobile-first throughout. Installable to the home screen on iPhone and Android without an app store. Designed for users who are not comfortable with technology.
The takeaway
The most powerful health tool for a senior is not a wearable or a portal. It is a two-minute habit and a one-page printout. Vitality 365 is proof that the right product for a specific audience looks nothing like the generic version built for everyone.
We built the app around the doctor's appointment — not around data collection for its own sake. That single design decision is what made it work.
